Remove Your Information from Citizen Health or Unite Genomics

If you would like your information removed from Citizen Health or Unite Genomics, please follow the instructions below to withdraw your consent, close your account, and/or delete your personal information. Please note: Withdrawing stops your information from being shared going forward. Information that was already shared with researchers before you withdrew can't be taken back.

Citizen Health

• Email support@citizenhealth.com and ask to withdraw your research consent, advocacy sharing, and close your account.
• Before sending, download any medical records you want to keep from your profile. Once your account is closed, you won't be able to get them back.
• Your account and records will be deleted within 30 days.

Unite Genomics

• Email movr-study@unitegenomics.com to withdraw from the study.
• Email privacy@unitegenomics.com to close your account and delete your personal information.
• If you want to keep your records, download them from your account first.
