Activity Overview: 

In this webinar, Christina Trout, MSN, RN of University of Iowa, Areti Vassilopoulos, PhD, of Yale School of Medicine, and Aravindhan Veerapandiyan, MD of Arkansas Children's Hospital will review pediatric-to-adult care transition practices, including challenges and best peer-reviewed strategies when navigating and implementing a transition process with NMD patients.

Learning Objectives: 

Identify challenges to navigating pediatric-to-adult care transitions in NMD.  
Identify care center-specific solutions to transition challenges in NMD.  
Evaluate opportunities to adjust current procedures to include best practices from exemplary institutions.
